He has led many redesign projects for care coordination in hospitals and healthcare systems. He is a published author of various care coordination articles and the book entitled Prevent Denials and Win Appeals: The Hospital Case Manager's Guide to Revenue Integrity

Examples of Outcomes

Transitional Care/BPCI/ Readmission Outcomes

 

Development of Programs with reduction of readmissions at small to large academic medical centers leading to improvements as much as 21.5% to 15.8% in just 3years with elimination of penalties. Additionally, the TCM program was tied to BPCI that generated over $5 million dollars in revenue in the same timeframe.

 

Additional outcomes including reducing sepsis readmissions through an NP led TCM model from a 30% readmission rate to a 15.4% rate along with driving mortality down during the same 2-year period. 

 

Length of Stay Reduction

 

Care Coordination/Case Management model redesign and enhancement has led to many savings and reduction in LOS. Models have included dyad and triad design. Each facility or health system is unique so it can require a different approach. Some of the outcomes included reducing LOS from 6.5 days to 5 days at an academic medical center to exceeding the 85th percentile for a severity adjusted LOS compared to other large 800+ bed facilities with a reduction of LOS by 0.5 days in just 6 months that included revenue of over $7.8 million dollars. 

 

LOS reduction was also accomplished at other facilities with implementation of complex care programs with emphasis on high LOS patients with multiple barriers. At one inner city facility with over a 78% Medi/Medi payer mix a reduction of LOS from 21 days on average to 16 days was accomplished with this method and a reduction of cost of over $1 million dollars per day. 

 

Denial Reduction

 

Average Denial Reduction at most locations is around 3% which generally leads to revenue of $7 to $12 million net per year. It can lead to more depending on discharges and adjusted net revenue per discharge. UR/UM strategies are a major strength of our consultation including having published a book on this subject and many articles and providing lectures across the U.S. to many audiences.
